Add additional layer of security to actions done by Server Admin interfaces

Requirements:

1. Applied to any Server Admin pages with executable actions and to Gogo Shell portlet

2. Captcha behavior should mirror other usages in DXP

   1. In both Success and Failure, appropriate message should be returned in the UI, the Captcha input field is cleared, and any other user input fields are NOT cleared

3. Captcha is always required before any action is taken. MaxChallenges is not applied.
